Objective

The integrity and cosmesis of vulvar wound closure after vulvectomy are maximized when there is minimal tension and a good blood supply to the closed wound. The goal of this video is to show that V-Y fasciocutaneous advancement flaps can cover large vulvar defects that extend near the anal opening.

Description

The surgical treatment of vulvar cancer and other non-malignant vulvar lesions is often radical vulvectomy or wide local excision. For the surgeon, the defect after this type of surgery is large and difficult to repair. For the patients, these defects are disfiguring and add to the stress of coping with the underlying disease. These stressors may add to the high incidence of sexual dysfunction and depression in patients with these conditions. The V-Y fasciocutaneous advancement flap technique

Conclusion

The V-Y fasciocutaneous advancement flap technique is ideal for closure of these large defects as it preserves good blood supply to the surrounding skin. In addition, it provides an excellent cosmetic result. This procedure is easy to learn and adds to the surgical armamentarium of the gynecologic surgeon.
