Introduction
La radiothérapie mammaire, avec ou sans irradiation des aires ganglionnaires, est indiquée dans la prise en charge adjuvante des cancers du sein après chirurgie conservatrice et, en cas de facteurs de pronostic défavorable, après chirurgie non conservatrice.
La radiothérapie médiastinale, associée à une chimiothérapie combinant la doxorubicine, la bléomycine, la vinblastine et la dacarbazine (ABVD) ou la bléomycine l’étoposide, l’adriamycine, le cyclophosphamide, l’oncovin, la prednisone et la procarbazine (beacopp) est la prise en charge de référence de la maladie de Hodgkin.
Une irradiation en conditions stéréotaxiques pour une lésion pulmonaire, médiastinale ou vertébrale dans le cadre du traitement d’une pathologie primaire ou secondaire thoracique est devenue un traitement fréquent.
La réalisation d’une irradiation de ces localisations délivre nécessairement une dose dans le cœur, qui, associée aux facteurs de risque cardiovasculaire propres au patient et au reste de son traitement potentiellement cardiotoxique, va potentiellement générer une toxicité cardiaque.
L’objectif de cette revue de la littérature est de rapporter la toxicité cardiaque induite par la radiothérapie conformationnelle tridimensionnelle, normofractionnée avec modulation d’intensité (RCMI) ainsi qu’en conditions stéréotaxiques et d’évaluer les moyens à notre disposition pour limiter la cardiotoxicité, et enfin de faire la synthèse des recommandations sur les contraintes de doses dans le cœur et les artères coronaires.

Toxicité cardiaque et radiothérapie
L’ensemble des facteurs potentiels de cardiotoxicité au cours de la prise en charge du cancer du sein sont repris dans l’étude de Chargari et al. [1]. Les facteurs potentiels de cardiotoxicité de la maladie de Hodgkin et du cancer bronchopulmonaire sont résumés dans les Fig. 1, Fig. 2.
La toxicité cardiaque a été largement étudiée dans les cancers du sein, ainsi que dans les lymphomes de présentation médiastinale.

Toxicité cardiaque et radiothérapie en conditions stéréotaxiques
La radiothérapie en conditions stéréotaxiques implique par définition l’administration de hautes doses par fraction avec une grande précision. La question se pose alors des effets tardifs potentiels liés à cet hypofractionnement. Les fortes doses par fraction entraînent une dévascularisation, une dénervation, et une dévitalisation des tissus menant à la fibrose, la sténose ou l’ulcération. Ces généralités sur la toxicité liée à la stéréotaxie peuvent être appliquées au risque de toxicité
Recommandation en radiothérapie conformationnelle tridimensionnelle
Les contraintes de doses issues de la littérature sont retranscrites dans le Tableau 2. Un volume recevant 25 Gy de moins de 10 % est insuffisant pour assurer un risque de mortalité cardiovasculaire de moins de 1 %, la limite de cette contrainte est la possibilité de laisser 9,9 % du cœur recevoir 50 Gy. Nous avons vu que le risque cardiaque est aussi lié aux doses reçues par les différentes sous-structures, non prises en compte par ces contraintes. En l’absence de recommandation chiffrée par
Prise en compte en priorité de la dose reçue par le muscle cardiaque ou par les artères coronaires ?
La dose dans les artères coronaires semble avoir un impact à part entière pour la toxicité cardiaque, particulièrement pour le risque d’infarctus du myocarde. Cependant, la difficulté de délinéation précise et reproductible des artères coronaires en pratique courante et la grande variabilité interindividuelle, même avec l’aide de recommandations internationales rend impossible à l’heure actuelle d’établir des contraintes de doses aux artères coronaires à utiliser en pratique courante.
