Characterization of neurotensin(6–13) from an hepatic fibrolamellar carcinoma

Abstract

Neurotensin(6–13) has been isolated and sequenced as the major form of neurotensin-like immunoreactivity (NTLI) in a human hepatic fibrolamellar carcinoma. Circulating NTLI in the patient, especially C-terminal, was very high. In additional studies, NT(6–13) was synthesized and compared with the purified tumor NTLI by HPLC analysis and by testing stability in plasma in vitro. These methods confirmed that the tumor NTLI was identical to NT(6–13). Since the metabolic clearance rate of synthetic NT(6–13) in sheep was 30-fold higher than NT(1–13), it suggests that the elevated plasma levels are the result of impaired clearance and/or markedly elevated production.

      The gene encoding NTS is also spatio-temporally regulated during human development with a transient expression in the fetal liver, colon, pancreas, and stomach, suppressed in the adult within the first weeks of birth [101]. Re-expression has been reported in the rare fibrolamellar hepatic carcinomas [102,103], in one sample of hepatocellular cancer [104], and in approximately 25% of resected colon cancers [105]. The pathological NTS expression in ectopic tissues suggested modifications in the regulatory mechanisms of the NTS gene.

      Answering this question is of significance insofar as it is still not settled whether the pathways leading to FL-HCC are similar or clearly different from those resulting in adult-type HCC, even though FL-HCC is, apart from its distinct morphology and predominant manifestation age, strikingly different from ordinary HCC in terms of several other aspects. These aspects include the development in noncirrhotic livers, a better prognosis,8 rarity of high-titer serum alpha-fetoprotein, neuroendocrine features,5 aromatase activity of tumor cells,6 marked expression of the invasion-related membrane protein CD44,10 a diploid DNA pattern,11 and nearly normal karyotypes, contrasting with the numerous aberrations detected in other HCCs.12 In the event that tumor components resembling FL-HCC are detectable within an adult-type HCC, it is difficult to judge by conventional histology alone whether such sclerosing lesions in fact represent bona fide FL-HCC or rather a stromal-rich subline emerging in an adult-type HCC, the latter tumors being well known for their heterogeneity in composition, also with respect to matrix production.
