The first to study this was Patti and colleagues,34,35 who performed dual-probe 24-hour pH monitoring studies on patients with chronic respiratory symptoms of unknown origin with the goal of identifying aspirating patients. In this study, aspiration was defined by respiratory symptoms that occurred within 3 minutes after a reflux episode detected by both proximal and distal pH probe.

Respiratory symptoms are well-recognized extra-esophageal manifestations of gastroesophageal reflux,63 and reflux extending into the proximal esophagus and cricopharyngeal region is thought to reflect a high risk for microaspiration;44,64,65 however, its sensitivity and specificity are unknown. The measurement of pepsin and bile salt in BAL may help clarify this issue.
