Spontaneous spasm of native coronary arteries and saphenous vein bypass grafts has previously been demonstrated by coronary angiography.1,2 Direct observation of spasm involving internal mammary artery (IMA) bypass graft has been reported in the immediate postoperative period.3 We present angiographic documentation of spasm in left IMA bypass graft in the late postoperative period.
