Organisation de l’étude
Douze pédiatres, exerçant en pratique libérale dans le département du Val-de-Marne ont conduit cette étude ouverte, randomisée, comparative. Avant de débuter l’étude, le protocole clinique a été revu et approuvé par le CCPPRB du centre hospitalier universitaire Henri Mondor et il a été soumis à l’agence du médicament conformément à la réglementation française. Préalablement à l’inclusion dans l’étude, les parents ou tuteurs de chaque nourrisson étaient informés de l’étude et de leur rôle au
Résultats
Les résultats de l’analyse d’immunogénicité « per protocole », ainsi que les résultats de l’analyse de tolérance sont présentés dans cet article. Les nourrissons qui ont reçu au moins une dose de vaccin(s) à l’étude ont été inclus dans l’analyse de tolérance.
Au niveau des 11 centres investigateurs, 160 nourrissons ont été inclus et randomisés afin de recevoir le(s) vaccin(s) à deux, trois et quatre mois. Sur la demande des parents, trois nourrissons ont été exclus de l’étude avant
Discussion
Depuis la disparition des méningites à Haemophilus influenzae type b grâce au vaccin conjugué, la méningite à pneumocoque a pris une place prépondérante : ainsi sur près de 200 méningites à Pneumocoque observées chaque année en France chez l’enfant de moins de 16 ans, une majorité l’est chez le nourrisson et le jeune enfant, l’âge médian de survenue étant un an [7], [12].

In children aged 2–15 months, PCV7 with other childhood vaccines provided adequate antibody response when administered concomitantly [36–41]. It has also been observed that serotypes 6B, 23F and 9V are less immunogenic after completion of the 3-dose scheme; however, antibodies for serotypes 6B and 23F seem to rise following the administration of a booster dose [26].

2008, VaccineCitation Excerpt :In infants who received the primary immunization at 2, 3 and 4 months of age with the pneumococcal vaccine, antibody concentrations for all seven serotypes increased substantially. GMC values for all seven serotypes after dose 3 are similar to those reported in four previous studies conducted in Germany and/or France, where PCV7 was administered with the same schedule concomitantly to other recommended paediatric vaccines [32,33,43,44], except for serotype 6B for which GMC values were higher in 3 of these studies (2.5–3.2 μg/mL vs. 1.3) [32,43,44]. In our study, about 92% of subjects achieved serotype 6B antibody levels ≥0.15% and 80% antibody levels ≥0.50 μg/mL after the third dose of the primary series.

Both schedules result in comparable antibody concentrations, an observation which was confirmed in the studies by Tichmann-Schumann et al., Reinert et al., Schmitt et al. and Choo et al. [14–16,19] (Table 1). Two clinical trials investigated the immunogenicity after a priming series of two doses of PCV7 instead of three.
