Introducción
La cirugía laparoscópica representa una alternativa segura, igual de eficaz y menos invasiva en un buen número de procedimientos quirúrgicos frente a las técnicas tradicionales abiertas1,2. Pacientes y métodos
Previamente al inicio, se obtuvo la aprobación del Comité de Ética hospitalario así como el consentimiento informado de cada uno de los pacientes. El diseño del estudio fue el de un ensayo clínico aleatorizado simple ciego. Se incluyeron pacientes ASA I y II programados para colecistectomía laparoscópica electiva. Resultados
De los 30 pacientes incluidos en el estudio, 1 fue excluido a posteriori por inclusión errónea (cumplir un criterio de exclusión). Los datos antropométricos y demográficos de los 29 pacientes analizados, así como las características referentes a la técnica laparoscópica (presión intrabdominal creada y duración del neumoperitoneo) y variables basales (presión arterial media y estimación del aclaramiento de creatinina según la fórmula de Cockroft-Gault) se muestran en la Tabla 1 Las diuresis
Discusión
En este estudio se observa una disminución de la filtración glomerular y de la diuresis durante las colecistectomías laparoscópicas siendo mucho más pronunciado cuando la reposición volémica se realiza exclusivamente con cristaloides (Ringer lactato). Los resultados obtenidos, como la disminución de la perfusión de los órganos intraperitoneales, fueron los esperables de acuerdo con los efectos fisiopatológicos del neumoperitoneo1,2,11,12. Hydroxyethyl Starch or Saline for Fluid Resuscitation in Intensive Care

Our post hoc analysis showed that treatment with HES was associated with increased urine output in patients with less severe acute kidney injury, which may have been due to increased intravascular volume or through a diuretic effect.19 Conversely, serum creatinine levels were consistently higher in the HES group, suggesting a progressive reduction in creatinine clearance and more severe acute kidney injury.
