Imaging findings of adult bochdalek hernias

Abstract

The purpose of this article is to review the computed tomographic (CT) and related radiographic findings of adult Bochdalek hernias. Bochdalek hernias are one of the most common congenital abnormalities encountered in imaging studies of the chest and abdomen. CT can be used to confidently diagnose Bochdalek hernias, which can easily mimic neoplasms on other imaging studies.

    In the post natal evaluation of diaphragmatic hernia, ultrasonography can assist in delineating the diaphgram in its entirety and also in evaluating the viscera that has herniated into the thoracic cavity [7]. Computed Tomography is believed to be the most accurate method of diagnosing and evaluating the contents of these hernias especially the smaller ones [3,8]. CT with multiplanar reconstructions can accurately assess the thoracic cavity and also detect any other associated anomalies like hypoplasia of the right lobe of the liver as was seen in our case.

    A chest roentgenogram demonstrating gas and fluid-filled viscera above the diaphragm can support the diagnosis of Bochdalek hernia. Other more subtle findings include blunting of the costophrenic angle, presence of a posterior mediastinal mass, and small pleural effusion [4]. Computed tomography can also be diagnostic.
