Bei einem 75-jährigen Patienten wurde vor einem Jahr bei wiederholt aufgetretenen massiven bifrontalen Kopfschmerzen (10/10) ein Hypophysentumor diagnostiziert. Die Kopfschmerzen waren von Benommenheitsschwindel, Übelkeit und Erbrechen begleitet, von einer deutlichen Photophobie sowie mit vertikal versetzten Doppelbildern assoziiert. Endokrinologisch war eine partielle Hypophysenvorderlappeninsuffizienz mit Entkoppelungshyperprolactämie mit erniedrigtem basalen Kortisol (durch ACTH noch grenzwertig stimulierbar) nachgewiesen. Im Rahmen einer stationären internistischen Behandlung bei hypertensiver Krise trat dann eine Abducens- und Okulomotoriusparese rechts auf. Die damals durchgeführte Kernspintomografie hatte den Verdacht auf einen Vertebralisverschluss rechts und eine etwa 50%ige Vertebralisstenose links ohne Diffusionsstörung ergeben. …
