Zusammenfassung
Die Fortschritte in der Nierenersatztherapie haben ein Langzeitüberleben von Kindern mit chronischer Niereninsuffizienz ermöglicht. Heute sind es vor allem Langzeitkomplikationen, die eine normale Lebenserwartung und eine altersentsprechende Lebensqualität noch deutlich einschränken. Ein Teil dieser Langzeitprobleme ist durch das Auftreten der Nierenerkrankung im Kindesalter und durch extrarenale Organbeteiligung erklärt, ein anderer Teil durch spezielle krankheitsbedingte Langzeitkomplikationen und durch Probleme der langjährigen Nierenersatztherapie. Schulische Ausbildung, Berufsfindung und insbesondere die soziale Rehabilitation sind häufig beeinträchtigt. Besondere Bedeutung für das Langzeitüberleben hat die kardiovaskuläre Komorbidität.
Abstract
Advances in renal replacement therapy have made long-term survival in pediatric and adolescent patients with chronic kidney disease possible. However, long-term complications continue to restrict life expectancy and quality of life in these patients. Some of these long-term problems are caused by the onset of chronic kidney disease in childhood or by extrarenal organ involvement, while others are caused by particular disease-associated long-term complications as well as long-term renal replacement therapy. Education, career and in particular social rehabilitation are often adversely affected. The extent of cardiovascular comorbidity is of major importance for long-term survival.
