Zusammenfassung
Bei der Dehydratation eines Kindes ist auf die Schwere der Exsikkose und auf Zeichen eines Volumenmangelschocks zu achten, da sich daraus die weiteren Maßnahmen ableiten. Bei milder bis moderater Dehydratation kann die Therapie in der Regel mit oraler Rehydratationslösung erfolgen. Bei schwerer Exsikkose ist eine intravenöse Volumensubstitution unter Beachtung der Elektrolyte notwendig. Für den initialen Volumenbolus und den weiteren Flüssigkeitsausgleich eignen sich glukosefreie balancierte Vollelektrolytlösungen. Insbesondere eine Hypo- oder Hypernatriämie muss unter engmaschigen Kontrollen ausgeglichen werden, um neurologische Komplikationen zu vermeiden, je schwerer die Ausprägung, desto vorsichtiger. Bei jeder Form der Dehydratation ist eine regelmäßige Evaluation des Kindes wichtig – mindestens nach jeder durchgeführten Maßnahme. Dabei werden Bewusstsein, Hautturgor, Kapillarfüllungszeit, Herzfrequenz, Blutdruck, Diurese und Körpergewicht sowie Elektrolyte, Blutzucker und Laktatspiegel beurteilt.
Abstract
In cases of dehydration in children attention must be paid to the severity of the exsiccosis and to signs of hypovolemic shock, because these determine the subsequent measures. In cases of mild to moderate dehydration treatment can be carried out with oral rehydration solutions. In cases of severe exsiccosis intravenous volume substitution taking the electrolytes into account is necessary. Glucose-free balanced electrolyte solutions are suitable for the initial volume bolus and further fluid substitution. Hyponatremia and hypernatremia, in particular, must be carefully balanced under close monitoring in order to avoid neurological complications: the more severe the expression, the more care must be taken. For every form of dehydration, a regular evaluation of the child is important, at least after performing every measure. This includes assessment of consciousness, skin turgor, capillary filling time, heart rate, blood pressure, diuresis and body weight as well as electrolyte, blood glucose and lactate levels.
