Abstract
Background
Acute appendicitis is one of the main pathological conditions requiring emergency surgical intervention. The most widely accepted scoring system is modified Alvarado scoring system (MASS). In this study we aimed to improve the efficiency of MASS by adding a new parameter and to evaluate its efficiency in the diagnosis of acute appendicitis.
Methods
This study included 158 patients who underwent acute appendectomy in Keçiören Training and Research Hospital General Surgery Department. In addition to criteria of MASS, all patients were questioned about the presence of tenesmus. The validity of MASS and MASS with additional parameter was evaluated with respect to sensitivity, specificity and positive and negative predictive values.
Results
Accuracy rates of MASS, clinical findings, ultrasonography and MASS with additional parameter in the diagnosis of acute appendicitis were 64, 76, 85 and 80 %. False positivity rates for clinical findings, MASS and MASS with additional parameter in the diagnosis of acute appendicitis were 17, 26 and 10 %, respectively. Sensitivity and specificity of clinical findings in the diagnosis of acute appendicitis were 83 and 66 %, respectively. Sensitivity and specificity of MASS in the diagnosis of acute appendicitis were 74 and 39 %, respectively, and those of MASS with additional parameter were appendicitis increased to 83 and 66 %, respectively.
Conclusions
MASS is a simple, cheap and objective scoring system and does not require expertise. When tenesmus is added to standard MASS, rates of accuracy, sensitivity and specificity become better than those in MASS in the diagnosis of acute appendicitis.

Grundlagen
Die akute Appendizitis ist einer der häufigsten Gründe für Notoperationen. Der am meisten anerkannte Score zur Diagnosestellung ist das modifizierte Alvorado Score System (MASS). Ziel der vorliegenden Studie war es, die Effizienz des MASS durch Einbeziehung eines neuen Parameters zu verbessern.
Methodik
In diese Studie wurden 158 Patienten eingeschlossen, die einer akuten Appendektomie an der Abteilung für allgemeine Chirurgie des Keçiören Training- und Forschungs-Spitals unterzogen worden waren. Zusätzlich zu der Erhebung der Kriterien des MASS wurden alle Patienten nach dem Vorliegen von Tenesmus gefragt. Die Treffsicherheit von MASS und von MASS mit diesem zusätzlichen Parameter wurde durch Erhebung der Sensitivität, der Spezifität und der positiven und negativen Voraussagewerte erhoben.
Ergebnisse
Die Treffsicherheit in Bezug auf die Erkennung einer akuten Appendizitis von MASS lag bei 64 %, von der Klinik bei 76 %, vom Ultraschallergebnis bei 85 % und von MASS mit dem zusätzlichen Parameter bei 80 %. Falsch positiv war die Klinik in 17 %, das MASS bei 26 % und das MASS mit dem zusätzlichen Parameter bei 10 %. Die Sensitivität im Bezug auf die korrekte Diagnose akute Appendicitis war für die Klinik 83 %, die Spezifizität 66 %. Die Sensitivität vom MASS 74 %, die Spezifizität 39 %. Für das MASS mit zusätzlichem Parameter ergab sich eine Sensitivität von 83 %, beziehungsweise eine Spezifizität von 66 %.
Schlussfolgerungen
MASS ist ein einfaches, billiges und objektives Scoring System, das keine besondere Expertise benötigt. Die zusätzliche Einbeziehung der Frage, ob ein Tenesmus vorliegt, verbessert die Treffsicherheit, Sensitivität und Spezifizität von MASS in Bezug auf die Erkennung einer akuten Appendizitis.
