Zusammenfassung
Hintergrund
Die Keratoconjunctivitis vernalis (VKC) und die atopische Keratokonjunktivitis (AKC) sind komplexe und seltene Krankheitsbilder. Daher sind ihre Diagnose und Therapie oft eine Herausforderung.
Fragestellung
Es erfolgen die Erörterung der Epidemiologie, neuer pathogenetische Konzepte, interessanter klinischer Gesichtspunkte, diagnostischer Möglichkeiten und neuer Therapieoptionen und deren Nebenwirkung bei schweren okulären Allergien sowie eine Analyse der Darstellung der Erkrankung „Keratoconjunctivitis vernalis“ im Internet.
Material und Methode
Aktuelle Übersichtsarbeiten, Originalarbeiten und Falldarstellungen zum Thema VKC und AKC der letzten 5 Jahre werden evaluiert.
Ergebnisse
Okuläre Allergien haben über die letzten Dekaden signifikant zugenommen. Aktuelle Konzepte, die in der Pathogenese von VKC und AKC diskutiert werden, sind die Rolle des lokalen und Darmmikrobioms sowie der Einfluss der Neuroinflammation. Ein Keratokonus ist bei Patienten mit VKC und AKC signifikant häufiger als in der Normalpopulation. Er ist mit rascherer Progredienz sowie schwererem Verlauf assoziiert. Selten ist in der Diagnose der allergischen Konjunktivitis ein Bindehautprovokationstest notwendig. Die Therapie der atopischen Dermatitis mit dem IL(Interleukin)-4Ra-Antagonisten Dupilumab kann zu okulären Nebenwirkungen führen, die schwer von einer atopischen Keratokonjunktivitis zu unterscheiden sein können. Leider sind die für Patienten und Eltern im Internet verfügbaren Informationen zum Thema VKC teilweise gefährlich falsch.
Schlussfolgerungen
Aus den oben beschriebenen neuen pathogenetischen Konzepten könnten sich in Zukunft präventive und personalisierte Therapieoptionen entwickeln. Ein Keratokonus bei AKC/VKC muss frühzeitig erkannt und behandelt werden. Für die Durchführung eines standardisierten Bindehautprovokationstests zur Diagnose einer allergischen Konjunktivitis stehen jetzt aktuelle Leitlinien zur Verfügung. Die okulären Nebenwirkungen von Dupilumab sind oft schwer von der eigentlichen atopischen Keratokonjunktivitis zu unterscheiden und sprechen gut auf eine antientzündliche Therapie an. Patienten mit VKC müssen auf die Falschinformationen im Internet bezüglich ihrer Erkrankung hingewiesen werden.
Abstract
Background
Vernal keratoconjunctivitis (VKC) and atopic keratoconjunctivitis (AKC) are complex and rare diseases. Thus, their diagnosis and treatment are often a challenge.
Objective
Discussion on the epidemiology, new pathogenetic concepts, interesting clinical findings, diagnostic possibilities and new treatment options and their side effects in severe ocular allergies. Analysis of the presentation of VKC in the internet.
Material and methods
Evaluation of recent review articles, original publications, and case reports on the topics of VKC and AKC over the past 5 years.
Results
Ocular allergies have significantly increased over the last decades. Recent concepts discussed in the pathogenesis of VKC and AKC are the role of the local and gut microbiome as well as the influence of neuroinflammation. Keratoconus is significantly more common in patients with VKC and AKC compared to the normal population. It is associated with faster progression and a more severe course of disease. A conjunctival provocation test is only rarely necessary in the diagnosis of allergic conjunctivitis. Treatment of atopic dermatitis with dupilumab, an interleukin 4 receptor alpha (IL-4Ra) antagonist, can cause ocular side effects. Unfortunately, information available on the internet for patients and parents on the topic of VKC is sometimes dangerously incorrect.
Conclusion
From the abovementioned new pathogenetic concepts, preventive and personalized treatment options could be developed in the future. Keratoconus in AKC/VKC must be recognized and treated early. Official guidelines are now available for a standardized conjunctival provocation test in the diagnosis of allergic conjunctivitis. The unwanted ocular side effects of dupilumab are often difficult to discriminate from the actual underlying AKC and respond well to anti-inflammatory treatment. Patients with VKC must be informed about the incorrect information on the internet regarding their disease.
