Zusammenfassung
Hintergrund
Die Zentrierung der Ablationszone hat einen entscheidenden Einfluss auf das Ergebnis der wellenfrontgeführten LASIK. Zyklorotationen des Auges können beim Wechsel von der sitzenden Position des Patienten während der Aberrometrie in die liegende Position während der Laserbehandlung auftreten und Aberrationen niederer und höherer Ordnung induzieren.
Methode
20 Patienten (40 Augen) erhielten eine wellenfrontgeführte LASIK (B&L 217z 100 Excimer Laser) mit Iriserkennungstracker (mittleres präoperatives SÄ: −4,72±1,45 dpt; Spannweite −1,63 bis –7,00 dpt). Während der Aberrometrie und nach Durchführung des Flapschnitts wurde die Irisstruktur des Patientenauges aufgenommen.
Ergebnisse
Im Mittel betrugen die Absolutwerte der gemessenen Zyklorotationen −1,5±4,2° (−11,0 bis 6,9°). Der Betrag der Zyklorotationen ergab im Mittel 3,5±2,7° (0,1 bis 11°). Eine Zyklorotation >2° trat in 65% der Augen auf.
Schlussfolgerung
Die Iristracker-Technologie zeigte, dass es zu Zyklorotationen von bis zu 11° während der Veränderung von sitzender zu liegender Position des Patienten kommen kann. Die Anwendung statischer Eyetracker mit Iriserkennung ermöglicht durch Erfassung und Ausgleich von Zyklorotationen des Auges die genauere Zentrierung des Ablationsprofils.
Abstract
Purpose
Centration of the ablation zone decisively influences the result of wavefront-guided LASIK. Cyclorotation of the eye occurs as the patient changes from the sitting position during aberrometry to the supine position during laser surgery and may lead to induction of lower and higher order aberrations.
Methods
Twenty patients (40 eyes) underwent wavefront-guided LASIK (B&L 217z 100 excimer laser) with a static eyetracker driven by iris recognition (mean preoperative SE: –4.72±1.45 D; range: −1.63 to –7.00 D). The iris patterns of the patients’ eyes were memorized during aberrometry and after flap creation.
Results
The mean absolute value of the measured cyclorotation was −1.5±4.2° (range: −11.0 to 6.9°). The mean cyclorotation was 3.5±2.7º (range: 0.1 to 11.0°). In 65% of all eyes cyclorotation was >2º.
Conclusions
A static eyetracker driven by iris recognition demonstrated that cyclorotation of up to 11° may occur in myopic and myopic astigmatic eyes when changing from a sitting to a supine position. Use of static eyetrackers with iris recognition may provide a more precise positioning of the ablation profile as they detect and compensate cyclorotation.
