Zusammenfassung
Einleitung
Häufige Ursachen der Arthrofibrose sind Bandoperationen bei posttraumatischem Reizzustand. Die Arthrofibrose ist die häufigste postoperative Komplikation nach vorderer Kreuzbandersatzplastik. Die dauerhafte Bewegungseinschränkung des Kniegelenks führt längerfristig zur Pangonarthrose. Eine frühzeitige Diagnosesicherung trägt zum Erfolg der Therapie bei.
Material und Methoden
Abhängig von der klinischen Ausprägung der Arthrofibrose kann zunächst der Versuch mit Physiotherapie unter Beachtung der endgradigen Kniestreckung und Patellamobilität durchgeführt werden. Therapeutisches Ziel ist das Erreichen einer annähernd physiologischen Beweglichkeit der Gelenkpartner. Falls dies mit konservativer Therapie nicht gelingt, ist die arthroskopische Arthrolyse erforderlich. Die Entfernung der Adhäsionen in allen Kompartimenten ist wichtig. Bei einem Rezidiv der Fibrose ist eventuell eine wiederholte arthroskopische Arthrolyse mit Notchplastik, Osteophytenresektion, Bridenentfernung und Releases zur Verbesserung der Beweglichkeit erforderlich. Bei erheblicher dorsaler Kapselfibrose, verbunden mit fibrotischem Verschluss der dorsalen Recessus, ist ein zusätzliches dorsales Release notwendig.
Schlussfolgerung
Die Prognose nach Arthrolyse hängt wesentlich von der erreichten Mobilität des Gelenks ab.
Abstract
Introduction
Ligament reconstruction of the knee during a posttraumatic inflammatory situation is a common cause of arthrofibrosis. Arthrofibrosis is the most frequent complication after anterior cruciate ligament reconstruction. Ongoing reduction of knee mobility leads to osteoarthritis and early diagnosis contributes to the success of therapy.
Methods
Depending on the clinical expression of arthrofibrosis conservative treatment with physiotherapy could be favored with respect to initial full extension and mobility of the patella and if this fails, arthroscopic arthrolysis is required. The removal of adhesions is important in all compartments. In severe cases affecting the posterior capsule a posterior capsule release is necessary. In cases of recurrence of stiffness several sessions of arthrolysis can be indicated. Therapeutically, an approximately physiological mobility of the joint including the patellofemoral joint should be achieved.
Conclusion
The prognosis after arthrolysis essentially depends on the achieved mobility of the joint.
