Zusammenfassung
Harnleiterstrikturen sind seltene Komplikationen einer Strahlentherapie, die oftmals spät erkannt werden. Ihre Folgen reichen von harmlosen Dilatationen des Harnleiters bis hin zum Verlust der Nierenfunktion und der potentiellen Gefahr einer lebensbedrohlichen Urosepsis.
Die Therapie der radiogenen Ureterstriktur ist eine anspruchsvolle Aufgabe für jeden Urologen. Als Optionen stehen unterschiedliche Operationstechniken von minimal-invasiven Verfahren über Harnleiterrekonstruktionen bis hin zum kompletten Ersatz des Harnleiters durch Darm zur Verfügung.
Dieser Artikel gibt eine Übersicht über die unterschiedlichen Therapieoptionen mit besonderem Schwerpunkt auf der Verwendung von Ileum- und Kolonsegmenten zum Harnleiterersatz.
Abstract
Ureteral strictures are uncommon complications of radiotherapy which are often recognized late. Their consequences range from harmless dilatation of the ureter to loss of renal function and potential life-threatening urosepsis.
Therapy of radiogenic ureteral stricture is a challenging task for every urologist. Several surgical strategies including minimally invasive procedures, reconstruction and partial or complete replacement of the ureter are available.
This article provides an overview of the various options in the treatment of radiogenic stricture of the ureter, focusing on the use of ileum and colon segments for ureteral substitution.
