Chancen und Grenzen der ambulanten Pädiatrie

Possibilities and limits of outpatient pediatrics

Zusammenfassung

Aufgrund der zunehmenden Komplexität der Kinder- und Jugendheilkunde ist eine Betreuung chronisch kranker Kinder im niedergelassenen Bereich in einer Einzelordination kaum mehr machbar. Durch Ärztezusammenschlüsse in Gruppenpraxen oder Primärversorgungszentren kann ein multidisziplinäres Team mit spezialisierten Kinderfachärzten und Kinderfachärztinnen die Betreuung auch von komplex erkrankten Patienten im Primärbereich übernehmen und somit wohnortnah anbieten.

Die Möglichkeiten und Grenzen werden für 3 Betreuungskonzepte am Beispiel eines Kindergesundheitszentrums aufgezeigt, und zwar die Nachsorgebetreuung von ehemaligen Frühgeborenen, den Aufbau einer Tracheostoma-Ambulanz und die Betreuung von allergopneumologischen Patienten. Diese Betreuungskonzepte sind für viele Bereiche der Kinderheilkunde möglich und bieten Vorteile für Arzt/Ärztin, Patienten und deren Familien sowie den Sozialversicherungsträger, da teurere Sekundär- und Tertiärzentren entlastet werden können.

Abstract

Due to the increasing complexity of pediatrics and adolescent medicine, the care of chronically ill children in a single private practice is practically no longer feasible. Joint efforts among group practices or primary care centers can provide a multidisciplinary team with specialized pediatricians to optimize the care of complexly ill patients in primary treatment and thus provide assistance close to home.

The possibilities and limitations of three concepts of care are illustrated exemplified by a pediatric healthcare center, including the follow-up care of preterm infants, the establishment of a tracheostomy outpatient clinic, and the care of patients with respiratory allergies. These care concepts are possible for diverse pediatric specialties and offer advantages for physicians, patients and their families as well as social insurance providers and ultimately reduce the pressure on more expensive secondary and tertiary centers.
