Zusammenfassung
Bei Dysrhaphien handelt es sich um Störungen des Neuralrohrschlusses (Neuralrohrdefekte, engl. „neural tube closure defects“, NTD). Hautveränderungen in der Mittellinie wie Hämangiome, Teleangiektasien, Pigmentanomalien, Hypertrichosis, Lipome, Hautfisteln (Dermalsinus) und Dermoidzysten können bereits im Neugeborenenalter Hinweis auf eine solche dysrhaphische Fehlbildung sein. Auch im späteren Alter lassen eine neurogene Blase und/oder neurologische Ausfälle das Vorliegen eines Defektes der Wirbelsäule vermuten. Auch bei neurologisch und orthopädisch noch unauffälligen Kindern muss in solchen Fällen eine Sonographie der Wirbelsäule und/oder eine Magnetresonanztomographie (MRT) des Spinalkanals veranlasst werden. Der Dermalsinus stellt eine neurochirurgische Notfallsituation dar, da die Gefahr einer aszendierenden Infektion besteht. Eine operative Sanierung ist deshalb zwingend erforderlich. Ein Tethered-cord-Syndrom kann als Folge aller dysrhaphischen Fehlbildungen, aber auch als Folge von Tumoren und Entzündungen entstehen. Bei der Darstellung der angeborenen Fehlbildungen (Dermalsinus, „tethered cord“, Spina bifida, Diastematomyelie, Syringomyelie, Chiari-II-Malformation etc.) und der Tumoren des Rückenmarks ist die MRT die diagnostische Methode der Wahl.
Abstract
Dysraphic malformations are congenital neural tube closure defects. Dermatologic findings in the dorsal midline, such as cutaneous hemangioma, telangiectasia, pigment anomaly, hypertrichosis, lipoma, dermal fistula (dorsal dermal sinus), and dermoid cyst, are clinical findings suggestive of a dysraphic malformation in an infant. In later childhood a neurogenic bladder and/or orthopedic malfunctions may point to a diagnosis of a defect of closure of vertebral arches. Even children who are still neurologically and orthopedically normal need to undergo an ultrasound of the spinal column and/or magnetic resonance imaging (MRI) of the spinal canal in those instances. The dorsal dermal sinus represents a neurosurgical emergency, as an ascending infection may develop. A tethered cord syndrome may be a result of all dysraphic malformations or may be caused by tumors and infections. MRI represents the imaging of choice to diagnose congenital defects such as dermal sinus, tethered cord, spina bifida, diastematomyelia, Chiari II malformation, etc.
