Wie ist die Gehörgangsanatomie und -physiologie?
Der äußere Gehörgang („external auditory canal“, EAC) ist eine gebogene Röhre von etwa 25 mm Länge bei Erwachsenen.
Äußeres Drittel besteht aus Knorpel/innere 2 Drittel aus Schläfenbeinknochen.
Was wissen Sie über die Ätiologie und Epidemiologie des Erkrankungsbildes?
Karzinome des äußeren Gehörgangs sind sehr selten (0,2 % aller Tumoren im Kopf- und Halsbereich).
Häufigste Tumorursache: Ausdehnung eines kutanen Malignoms der Ohrmuschel
Hauptrisikofaktoren:
– Helle Haut
– Exposition gegenüber ultraviolettem Sonnenlicht
– Immunsuppression
– Chronische suppurative Otitis media (CSOM)
– Typ 16 und 18 des humanen Papillomavirus (HPV)
– Zustand nach Radiatio
Welche diagnostischen Maßnahmen sollte man in die Wege leiten?
– Anamnese/Inspektion/Palpation des Halses
– Gewebebiopsie + histologische Analyse
– Hochauflösende Computertomographie („high-resolution computed tomography“, HRCT)/Magnetresonanztomographie (MRT)/ggf. Positronenemissionstomographie(PET)-CT
