Zur Tonsillektomiehäufung bei jungen Frauen

Hintergrund

Klinisch wirft die Häufung von Tonsillektomien bei jungen Frauen 2 Fragen auf: Ist dies statistisch zu sichern und, wenn ja, gibt es Hinweise auf eine möglicherweise psychologische Ursache dafür?

Material und Methoden

Die Tonsillektomiezahlen von 3 HNO-Kliniken und Literaturangaben wurden verglichen. Bei 24 Frauen im Alter von 15–20 Jahren erfolgte ein präoperatives Persönlichkeitsscreening mittels des Freiburger Persönlichkeitsinventars (FPI-R).

Ergebnisse

Die geschlechtsspezifische Häufung von Tonsillektomien bei weiblichen Patienten in der Altersgruppe 10–20 Jahre war statistisch signifikant. Die Persönlichkeitsuntersuchung zeigte in der Gesamtgruppe eine statistisch signifikante Erhöhung in den Subskalen soziale Orientierung und Aggressivität. 8 der 24 Patientinnen wiesen ein abnormes Persönlichkeitsprofil auf, wobei v. a. Korrelationen zwischen den Subskalen körperliche Beschwerden, Erregbarkeit und Aggressivität auffielen.

Fazit

Trotz der noch kleinen Stichprobe weisen diese Ergebnisse auf den Einfluss einer auffälligen Persönlichkeitsstruktur im psychosensiblen postpubertären Alter auf die Indikation zur Tonsillektomie hin.

Abstract

Background

The clinical observation of an accumulation of tonsillectomies in young women raises two questions: (1) Can this observation be proved statistically? and, if yes, (2) are there any hints as to a possible psychological reason of this phenomenon?

Material und methods

A total of 24 young females (15–20 years old) admitted to the ENT-department for tonsillectomy were given a preoperative personality screening using the Freiburger personality inventory (FPI-R).

Results

A significant, sex specific accumulation of tonsillectomies was found in females aged 10–20 years. The personality screening revealed a significant increase of the subscales "social orientation" and "aggressivity" in the entire group. Eight out of the 24 females showed an abnormal personality profile with statistically unexpected correlations between the subscales "somatic complaints", "excitability" and "aggressivity".

Conclusion

In spite of the low sample size, these results suggest that in some young females in the postadolescent age group, an abnormal personality structure influences the indication of tonsillectomy.
