Zusammenfassung

Der Lupus erythematodes ist eine prototypische Autoimmunerkrankung, die durch genetische Prädisposition und durch Umweltfaktoren induziert werden kann. Pathomechanistisch besteht eine unkontrollierte Aktivierung des Immunsystems, das fälschlicherweise körpereigene Strukturen erkennt und eine schubweise Entzündung induziert. Die multifaktorielle Genese basiert zumeist auf einem polygenen Vererbungsmodell, bei dem mehrere, häufige Genvarianten das Risiko der Krankheitsentstehung erhöhen. Viele dieser Polymorphismen sind erst in den letzten Jahren durch genomweite Assoziationsstudien identifiziert worden. Monogene Formen des Lupus erythematodes sind sehr selten. Ihre pathogenetische Aufklärung ist wesentlich für die Identifikation der Erkrankungsmechanismen, wie die Geschichte der Entdeckung von Komplementdefekten bei Patienten mit Lupus erythematodes zeigt. Der monogen autosomal-dominant vererbte familiäre Chilblain-Lupus ist durch kälteinduzierte Infiltrate an den Akren charakterisiert, die sich in der frühen Kindheit manifestieren. Die molekulare Aufklärung der Erkrankung zeigte, dass Defekte in der intrazellulären Elimination von Nukleinsäuren durch eine Typ-IFN-abhängige Aktivierung des angeborenen Immunsystems Autoimmunität und speziell einen Lupus erythematodes verursachen können. Dieser Mechanismus erweitert das Konzept der Lupuspathogenese: Nicht nur extrazelluläre, sondern auch intrazelluläre Defekte der Antigenelimination können zur Aktivierung des angeborenen und adaptiven Immunsystems führen.

Abstract

Lupus erythematosus is a prototypic autoimmune disease that can be triggered in genetically predisposed individuals by environmental exposures. The disease is based on an uncontrolled activation of the immune system that recognizes self antigens and induces inflammatory disease flares. The multifactorial pathogenesis is based on a polygenic model of inheritance with multiple various susceptibility genes elevating the disease risk. Many of these polymorphisms have been recently identified by genome-wide association studies. Monogenic forms of lupus erythematosus are rare. The identification of their underlying pathogenesis is important for the recognition of main mechanistic pathways in lupus as demonstrated by the history of defects in the complement system. The monogenic, autosomal dominant inherited familial chilblain lupus is characterized by cold-induced infiltrates on acral locations occurring in early childhood. Molecular exploration of the disease pathogenesis revealed that autoimmunity and especially lupus erythematosus can be induced by defects in intracellular elimination of nucleic acids and the subsequent type I-IFN-dependent activation of the innate immune system. This mechanism extends the concept of lupus pathogenesis: both defects in the extra- and intracellular elimination of autoantigens can lead to activation of the innate and adaptive immune system
