Zusammenfassung

Die Acne inversa ist eine chronisch-entzündliche Dermatose der Haarfollikel, die mit schwerer physischer und psychischer Belastung für die Patienten einhergehen kann. Klinisch ist sie durch subkutane, dolente Knoten, Abszessbildung und Fisteln bis hin zu Narbensträngen charakterisiert, wobei der Verlauf unbehandelt meist progredient verläuft. Die genauen pathogenetischen Hintergründe sind bislang weitgehend unklar. Konservative Therapieoptionen wie Antibiotika können eine Reduktion der Symptome bewirken, führen jedoch nicht zu einer Ausheilung. Therapie der Wahl, v. a. bei ausgedehnten Befunden, ist die radikale Exzision aller betroffenen Hautareale. Trotz aller verfügbaren Behandlungsmaßnahmen stellt die Acne inversa weiterhin eine große therapeutische Herausforderung dar.

Abstract

Acne inversa is a chronic inflammatory disease of the hair follicles. It can lead to severe functional and psychological impairment. It is characterized by inflamed painful nodules, abscesses, fistulas and scarring in late stages of the disease. The causes of acne inversa are still not fully understood. Conservative treatment options such as antibiotics may lead to clinical improvement; however they do not produce healing. Therapy of choice, especially in severe forms, is radical wide excision of all affected areas. Despite a variety of treatment options, acne inversa is still a therapeutic challenge.
