Konditionierung chronischer Wunden mittels proteolytischer Enzyme

Zusammenfassung

In den letzten Jahren wurde häufiger über den effektiven Einsatz verschiedener proteolytischer Enzyme in der Konditionierung chronischer Wunden berichtet. Proteolytische Enzyme in der Konditionierung chronischer Wunden ermöglichen eine weitestgehend schmerzfreie und selektive Nekrosektomie. Auch werden zahlreiche positive Effekte auf die Ausbildung einer rascheren Granulation und Epithelisation diskutiert. Somit könnte es durch die Wundgrundkonditionierung mit proteolytischen Enzymen zu einer Beschleunigung der Abheilung chronischer Wunden kommen. Trotz zahlreicher interessanter Teilaspekte sind diese Daten im Sinne einer evidenzbasierten Medizin noch nicht endgültig zu beurteilen. Für viele chronische Wunden stellt das enzymatische Débridement insbesondere in der ambulanten Therapie und ggf. in Kombination mit mechanischer Wundsäuberung heute eine Methode der 1. Wahl dar.

Abstract

In the past few years, several different proteolytic enzymes have been shown to be effective in conditioning the chronic wound beds. The use of proteolytic enzymes in wound-bed preparation enables a selective painless debridement. Moreover multiple positive effects on formation of granulation tissue and re-epithelialization have been observed. Therefore, a faster overall healing of chronic wounds may result. Moreover, numerous positive effects on a rapid induction of granulation are discussed. The supporting data do not fulfill the criteria of evidence-based medicine. For most chronic wounds today, enzymatic debridement represents a first choice method especially for outpatients and perhaps in combination with surgical debridement.
