Abstract
Introduction. The use of a venous port-catheter-system is known as a relatively safe implant. Besides infections, the breakage of PIPS is the most common reason for explantation before term. The purpose of this study is to analyse port-related complications and to show ways of preventingthem.
Method. Between 1.1.1994 and 31.12.1999, 391 PIPS were implanted in the V. subclavia with the Seldinger technique at Surgical Clinic 1 of the University of Leipzig. Subsequently, 311 of them were followed up until 31.12.2000, with a mean observation time of 45 months.
Results. We registered 48 complications altogether (15.4% of 311), 21(6.7%) of which occurred immediately after implantation (up to 30 days postoperatively).These could be divided either into wound-healing disorders/pulmonary distress (4.5%, n=14) or complications concerning the catheter systems (2.3%, n=7). Long-term complications after 31 days were evident in 27 patients (8.7%), due either to infections (4.5%, n=13) or catheter-associated problems (4.5%, n=14). Catheter lesions occurred in nine cases (2.9% out of 311) at the point of entry into the musculus pectoralis, i.e., where the catheter had to change direction. Typically these were lengthways tears caused by the catheter. We observed one full breakage without dislocation, and two dislocated catheter fragments in the systemic circulation. We consider the change of direction to be responsible for wear on the silicon catheter. During implantation, extreme change of direction of the catheter should be avoided because this is where breakage happens. Catheter implantation by means of exposure of the vena basilica in the infraclavicular triangle is the method of choice.
Zusammenfassung
Hintergrund. Die seit etwa 20 Jahren im Einsatz befindlichen venösen Portsysteme werden als ein relativ sicheres Implantat angesehen. Neben der Infektion zwingen am häufigsten Katheterokklusionen und Katheterbrüche zur vorzeitigen Explantation. Ziel der Arbeit war es, Portkomplikationen zu analysieren und Möglichkeiten zu deren Verhütung aufzuzeigen.
Methodik. Vom 1.1.1994 bis 31.12.1999 wurden an der Chirurgischen Klinik I der Universität Leipzig 391 Portsysteme in Seldinger-Punktionstechnik der V. subclavia implantiert. Bis 31.12.2000 wurden 311 Portsysteme nachbeobachtet. Die mittlere Nachbeobachtungszeit lag bei 45 Monaten.
Ergebnisse. Wir registrierten insgesamt 48 (15,4% von 311) Komplikationen, von denen sich 21 (6,7%) unmittelbar nach der Implantation (bis 30 Tage postoperativ) entwickelten. Diese rekrutierten sich aus 4,5% (n=14 von 311) Wundheilungs- und pulmonalen Störungen und zu 2,3% (n=7 von 311) aus Katheterproblemen. Spätkomplikationen ab 31. postoperativen Tag sahen wir bei 27 Patienten (8,7%), wobei 4,5% (n=13) Infektionen und 4,5% (n=14) Katheterprobleme aufwiesen. Katheterläsionen sahen wir in 9 Fällen (2,9% von 311) im Eintrittsbereich des Katheters in den M. pectoralis, d. h. an einem Richtungswechsel des Katheters. Typisch waren hierbei Längsrisse des Katheters. Wir beobachteten einen vollständigen Abriss des Katheters ohne Dislokation und zu 2 Katheterfragmentdislokationen in den Körperkreislauf. Wir sehen den Richtungswechsel des Katheters als Ursache für eine Materialermüdung des Silikonkatheters. Bei der Implantation ist peinlich auf die Vermeidung schroffer Richtungsänderungen zu achten, da diese massiv bruchgefährdet sind. Damit ist die offene Implantation des Katheters durch Freilegung der V. basilica in der Mohrenheim-Grube zu favorisieren.
