Zusammenfassung

Berichtet wird über einen Patienten mit einem „Man-in-the-barrel“-Syndrom aufgrund bilateraler Grenzzoneninfarkte bei Verschluss der linken A. carotis communis und Stenose der rechten A. carotis interna. Das klinische Bild ist gekennzeichnet durch eine rein motorische, proximal betonte, zentrale Paraparese der Arme. Pathophysiologisch liegt eine isolierte Läsion der Anteile der Pyramidenbahn zugrunde, die zu den Motoneuronen der Armmuskulatur deszendieren, was durch die Befunde der transkraniellen Magnetstimulation bewiesen wurde.

Abstract

A patient presenting with a “man-in-the-barrel” syndrome due to bilateral watershed infarcts as well as occlusion of the left common carotid artery and stenosis of the right internal carotid artery is reported. The clinical picture was characterized by a pure motor and proximally pronounced central paraparesis of the upper limbs. Pathophysiologically, an isolated lesion of the descending parts of the pyramidal tract to the upper limb motoneurons was confirmed by transcranial magnetic stimulation.
