Zusammenfassung
Hereditäre Amyloidosen stellen eine klinisch und genetisch heterogene Gruppe autosomal-dominant vererbter Erkrankungen dar. Sie werden durch Mutationen physiologischer Proteine verursacht; in den meisten Fällen handelt es sich um Transthyretin (TTR)-Amyloidosen. Die Art der Mutation bestimmt den Verlauf, die vorwiegend betroffenen Organe sowie den Altersgipfel der Erkrankung. Die Lebenszeit wird meist durch den Ausprägungsgrad der Kardiomyopathie begrenzt. Dargestellt werden die Kasuistiken zweier Männer, die plötzlich und unerwartet verstarben. Bei den Obduktionen konnte in beiden Fällen eine biventrikuläre Herzhypertrophie festgestellt werden. Die Diagnose einer kardialen Amyloidose erfolgte histologisch sowie molekulargenetisch. Eine frühe Diagnosestellung ist für die Erkrankten von großer Bedeutung, da die orthotope Lebertransplantation bis heute die einzige effektive Behandlung darstellt. Dies verdeutlicht auch den Nutzen einer Obduktion für die Hinterbliebenen, die ebenfalls erkrankt sein könnten.
Abstract
Hereditary amyloidoses present a clinically and genetically heterogeneous group of autosomal dominant diseases. The most frequent form is associated with mutations of the transthyretin gene. The type of mutation determines the process, the organs primarily involved as well as the time of onset of the disease. Life expectancy is generally limited by the degree of cardiomyopathy. The cases of two male patients who died suddenly and unexpectedly are presented. In both cases, autopsy revealed a biventricular cardiac hypertrophy. Cardiac amyloidosis was diagnosed by means of histologic and genetic analysis. Early diagnosis is essential for those affected, since liver transplantation still represents the only effective treatment. This illustrates the benefit of autopsy investigations for surviving relatives, who may themselves be affected by the disease.
