Zusammenfassung
Von September 1980 bis Februar 1992 wurden an unserer Klinik insgesamt 2100 Keratoplastiken durchgeführt. Bei 152 Augen von 145 Patienten waren perforierende Keratoplastiken wegen Hornhaut-Endothel-Epithel-Dekompensation (HEED) nach überwiegend extern implantierten Kunstlinsen erforderlich. Mittels prospektiver Erfassung und statistischer Auswertung der Patientendaten untersuchten wir Verlauf, Prognose und mögliche visus- und komplikationsbeeinflussende Faktoren. Insbesonders interessierten uns hierbei die Ergebnisse der 46 Augen, bei denen simultan nahtfixierte Hinterkammerlinsen (HKL) implantiert wurden. Die Nachbeobachtungszeit betrug durchschnittlich 15 Monate. Bei 77 der 152 Augen waren ursprünglich irisfixierte Kunstlinsen (IFL), bei 44 kammerwinkelgestützte Vorderkammerlinsen (VKL) und bei 31 Augen nach extra-kapsulärer Kataraktextraktion (ECCE) HKL implantiert worden. 8 der Kunstlinsen waren bereits zuvor entfernt worden, 39 wurden intraoperativ belassen, 61 entfernt und 52 ausgetauscht. Bei 46 Augen mit defektem Iris-Linsen-Diaphragma wurde nach IFL-oder VKL-Entfernung eine HKL sekundär implantiert und retropupillar entweder mittels zweier Irisnähte (32 Augen) oder mittels transskleraler Naht (14 Augen) fixiert. Bei 91% der Augen konnte eine Visus-verbesserung erzielt werden, der erreichte Visus war jedoch häufig durch persistierende zystoide Makulopathien beeinträchtigt. HEED nach extrakapsulärer Kataraktextraktion mit HKL hatten eine deutlich bessere Prognose (signifikant besserer Visus, signifikant seltenere zystoide Makulopathien) als solche nach IFL oder VKL. Die Augen mit sekundären nahtfixierten HKL zeigten im Vergleich zu den nach einer Kunstlinsenexplantation aphaken Augen ähnliche postoperative Verläufe, keine erhöhte Komplikationsrate und vergleichbare korrigierte postoperative Visusergebnisse.
Die Prognose der perforierenden Pseudophakie-Keratoplastik ist gut, der erreichte Visus jedoch häufig durch retinale oder Optikus-Veränderungen eingeschränkt. Die Implantation iris-oder skleranahtfixierter HKL nach Entfernung von IFL oder VKL erbringt bei offenbar nicht erhöhter Komplikationsrate für die Patienten eine schnellere und bessere optische Rehabilitation.
Summary
Between September 1980 and February 1992 we performed 152 penetrating keratoplasties in 152 eyes of 145 patients for pseudophakic bullous keratopathy among a total of 2100 keratoplasties in our institution. Patients were followed-up prospectively with an average follow-up of 15 months. We were specially interested in the outcome of 46 eyes in which posterior chamber lenses (PCL) were sutured into the eye. 77 of the implants were iris-supported lenses (ISL), 44 were anterior chamber lenses (ACL) and 31 were (PCL) following extracapsular cataract extraction (ECCE). In 132 of the eyes the intraocular lens (IOL) implantation had been performed elsewhere. 8 IOL had been removed earlier, and during penetrating keratoplasty, 39 intraocular lenses were left in place, 61 were removed and 52 were exchanged. Since 1988, in 46 eyes without capsular support, ACL or ISL were removed and PCL were implanted and sutured either to the iris (32 eyes) or transsclerally (14 eyes). After keratoplasty, visual acuity improved in 91% of the eyes. However, visual acuity was frequently limited by persisting cystoid macular edema. In eyes that had undergone keratoplasty following ECCE and PCL implantation, visual acuity was significantly better (p > 0.03) and cystoid macula edema was less frequently observed (p = 0.07) than in eyes with ISL or ACL. In eyes with sutured PCL complications appeared not to be more frequent and visual acuity appeared to similar to eyes that were left aphakic following IOL-explantation.
The prognosis of penetrating keratoplasty for pseudophakie bullous keratopathy is generally good, however, the final visual outcome is frequently limited because of retinal or optic nerve alterations. The implantation of iris-or transsclerally fixated PCL in 46 eyes was not accompanied by increased complications and allows faster and better visual rehabilitation.
