Résumé

Le couple endoscopie-biopsie permet actuellement la meilleure approche diagnostique dans le domaine des gastrites. Au plan analytique, les lésions élémentaires se présentent sous sept aspects: hémorragique, catarrhal, pâleur de la muqueuse, érosions, surélévations, augmentation du calibre des plis, rigidités. Ces anomalies peuvent être localisées ou diffuses, isolées ou groupées chez le même patient. Elles ne préjugent pas de l'état histologique L'interprétation en est difficile, et il faut tenir compte de l'âge, du contexte clinique, de l'évolution des lésions, des données biologiques et immunologiques. On peut alors distinguer les gastrites autonomes où l'atteinte muqueuse est le marqueur de la maladie; les gastrites spécifiques réactionnelles à une maladie générale; les gastrites associées à une pathologie gastro-duodénale ou hépato-biliaire.

Summary

The association endoscopy-biopsy allows at present the best diagnostic approach in the field of gastritis. As regards analysis, elementary lesions show 7 different appearances: hemorragic, catarrhal, pallor of the mucosa, erosions, elevations, thickening of the folds, rigidities. These abnormalities may be localized or diffuse, isolated or gathered in the same patient. They do not prejudge the histological state. The interpretation is difficult and account should be taken of age, clinical context, development of the lesions, biological and immunological data. It is then possible to distinguish autonomous gastritis where the mucosal lesion accounts for the disease, specific gastritis reactional to a systemic disease; associated gastritis to a gastro-duodenal or hepato-biliary pathology.
