Hyponatraemia and carbamazepine intoxication

Summary

The anticonvulsant, carbamazepine (Tegretol), has an anti-diuretic action. Serum sodium, osmolarity and carbamazepine blood levels were measured in 55 patients and related to symptoms of carbamazepine intoxication. Seven patients had hyponatraemia (serum sodium < 133mmol/l) and hypo-osmolarity; 5 of these were asymptomatic. Serum sodium was inversely correlated with carbamazepine blood levels and hyponatraemia occurred in older patients. Fifteen patients had symptoms of carbamazepine intoxication, 13 had normal serum sodium and osmolarity. Carbamazepine intoxication was considered not to be due to water intoxication.
