8. Zukunftsaussichten bei der Behandlung von Infektionen in der Chirurgie

Summary

Antibacterial therapy has today entered a vicious circle in which new resistant bacterial variants appear as rapidly as new substances are introduced for treatment. Since a strong selection element is inherent in the very concept of chemotherapy, the problems ahead will probably not be solved by enhancing the classical lines of chemotherapeutic research. Alternatives are, therefore, offered which avoid some disadvantages of classical chemotherapy. The most promising of these novel approaches are: 1. The use of immune sera with antibodies directed against LPS-“core” structures of gram-negative bacilli. 2. The development of drugs which stimulate host defenses. 3. The design and development of pharmacological methods to interfere with virulence properties of bacteria. 4. Pharmacological methods to discriminate against R-factor carrying bacteria.

Zusammenfassung

Die antibakterielle Chemotherapie befindet sich heute in einem Circulus vitiosus: Neue resistente Bakterien erscheinen fast so schnell wie neue Chemotherapeutica oder Antibiotica in die Chemotherapie eingeführt werden. Zur Lösung dieses Problems werden neue Konzepte benötigt, da bereits die ursprüngliche Idee der Chemotherapie mit dem Prinzip der Selektion resistenter Varianten verbunden ist. Eine durchgreifende Besserung der Situation in der antibakteriellen Chemotherapie ist deshalb nur von neuen Lösungen zu erwarten, von denen einige hier diskutiert werden: 1. Die Anwendung von Immunseren mit Antikörpern gegen rudimentäre LPS-Strukturen bei Gram-negativer Septikämie. 2. Die Entwicklung von Medikamenten, welche die Wirtsabwehr stimulieren. 3. Die Erfindung und Entwicklung pharmakologischer Methoden, mit denen man Virulenzeigenschaften von Bakterien abschwächen kann. 4. Die Entwicklung von Substanzen, die gegen die existierenden R-Faktoren selektieren.
