Summary

Treatment of splenic ruptures by the use of fibrin sealant and by sealant infiltration makes organ-preserving surgery generally possible. Partial resection (e.g. of cysts) is facilitated by the combined direct instillation and infiltration of the parenchyma with a sealant augmented by the application of collagen fleece. In hopeless cases, tangential splenic slices should be orthotopically reimplanted by fixation with fibrin sealant to obtain early vascularization. The different procedures are monitored by the use of modern diagnostics imaging techniques.

Zusammenfassung

Mit der direkten Fibrinklebung evtl. verbunden mit der Kleberinfiltration in das Parenchym ist bei Milzrupturen fast immer eine Organerhaltung möglich. Partielle Milzresektionen (z. B. bei Cysten) sind mit diesen Methoden unter zusätzlicher Anwendung von Kollagen-Vlies problemlos durchführbar. In aussichtslosen Fällen werden orthotop tangentiale Milzscheibchen reimplantiert, wobei die Fixierung mit Fibrinkleber eine wesentlich schnellere Revascularisation der Transplantate ermöglicht. Die Funktionskontrolle erfolgt mit szintigraphischen Methoden.
