Summary
A boy from Togo with a resistent Rhinoscleroma was observed. The specific granuloma was examinated by histological and electron microscopic methods. The plasma cells form fibrillary material or guttate Russel bodies in the rough endoplasmatic reticulum. The morphology of Mikulicz-cells and Klebsiella rhinoscleromatis is described. The cells are supposed to be (histiocytic) macrophages. The surface of the germs is coated with a fimbrialike radiate substance which in the vacuoles of the cells is crosslinked. This material could be the eliciting antigen for the formation of Russel bodies. These bodies then may represent immune substances bound within the cell.
Zusammenfassung
Bei einem Jungen aus Togo wurde ein therapieresistentes Rhinosclerom über 3 Jahre beobachtet. Histologisch und elektronenmikroskopisch konnte das spezifische Granulom untersucht werden. Die Plasmazellen bilden im rauhen endoplasmatischen Reticulum fibrilläres Material oder tropfige Russel-Körper. Deren strukturelle Bildung wird diskutiert. Die Mikulicz-Zellen und die Klebsiella rhinoscleromatis werden beschrieben. Die Zellen werden als (histiocytäre) Makrophagen angesehen. Die Erreger sind außen von einer fimbrienartigen, strahlig aufsitzenden Substanz in den Vacuolen der Zellen vernetzt. Dieses Material könnte den antigenen Reiz zur Bildung der Russel-Körper darstellen. Die Russel-Körper entsprechen vielleicht vergeblich gebildetem und gleichsam steckengebliebenem Immunmaterial.
