Summary
At the esophago-cardiac junction squamous epithelium is intermingled with gastric epithelium. Within the lamina propria of the distal 3 cm of the esophagus we found cardiac glands with many argyrophil cells in 60% of cases. Electron microscopy revealed seven well known types and two further forms which we could not identify: EC, ECL, D, D1; AL, I eg. M, S. It is supposed that these cells are modified neurotransmitters with a local activity. Their function is local regulation of the lower esophageal sphincter (LES).
Zusammenfassung
Bei 327 ösophagus-Kardia-PrÄparaten aus dem Obduktionsgut war das Plattenepithel des distalen ösophagus einoder mehrfach von Kardiadrüsenepithel unterbrochen, in 60% fanden sich in den distalen drei Zentimetern innerhalb der Lamina propria gro\flÄchige Kardiadrüsen. Hier waren immer argyrophile Zellen in nennenswerter Anzahl. Bei gleichzeitig durchgeführten elektronenmikroskopischen Untersuchungen von BiopsieprÄparaten dieser Region lag die Trefferquote bei 65%. Die elektronenmikroskopische Differenzierung anhand der Granulastrukturen brachte sieben definierte Zellarten (EC, ECL, D, D1, AL, I bzw. M, S) sowie zwei von uns nicht klassifizierbare Typen zutage. Die RegelmÄ\igkeit, mit der wir diese Zellen sowohl bei Erwachsenen als auch Neugeborenen und Kleinkindern gefunden haben, lÄ\t uns im Vergleich mit den biochemischen und physiologischen Ergebnissen zu diesem Themenkreis vermuten, da\ diese endokrinen Zellen am „Schauplatz des Geschehens” als Paraneurone bzw. peptiderge Neurotransmitter die Funktion des unteren ösophagussphinkter (zumindest mit-)regulieren.
