Summary
After intravenous injection the uptake of 99mTc as pertechnetate was measured in different tumours and brain in man. Tumour-brain concentration ratio for different tumours was 1.5–4.3∶1.
The sequential scintiphotography with a scintillation camera and computer analysis indicated a rapid diffusion of radioactive pertechnetate into the extracellular space of the tumour. A few min after injection, the relative concentration of 99mTc in the tumour in comparison to the brain was on the average utes 1.7∶1. These results suggest that blood-brain barrier disturbance in brain tumours is due to a pathological permeability of tumour vessels.
Zusammenfassung
Die Ausbildung eines möglichst hohen Tumor-Hirn-Quotienten ist für den szintigraphischen Nachweis von Hirngeschwülsten von entscheidender Bedeutung. Während tierexperimentelle Untersuchungen ergeben hatten, daß mit der Verbindung99m Tc-Pertechnetat Konzentrationsverhältnisse zwischen Tumor und normalem Hirngewebe von 12 bis 25∶1 zu erzielen sind, entsprechen die szintigraphischen Bilder hinsichtlich deutlicher Abgrenzung nicht immer den Erwartungen. Untersuchungen an operativ gewonnenem Tumormaterial ergaben mit Konzentrationsverhältnissen von maximal 4,3∶1 Relationen die den in vivo gemessenen Differenzen besser gerecht werden.
Für die Anreicherung der radioaktiven Substanz 99mTc-Pertechnetat ist ein metabolischer Prozeß unwahrscheinlich. Serienmessungen mit der Gammakamera und Auswertung über einen Digitalrechner ergaben, daß teilweise bereits nach 1 min Konzentrationsquotienten von durchschnittlich 1,7∶1 erreicht werden die bis zu 30 min keine wesentliche Änderung zeigten.
Es muß daher angenommen werden, daß die rapide Diffusion der radioaktiven Verbindung durch die insuffiziente Capillarwand der Tumorgefäße in den patholologisch erweiterten Extracellularraum des Tumors alleinige Grundlage der gegenüber dem normalen Hirngewebe vermehrten Konzentration ist. Der Versuch einer artdiagnostischen Differenzierung unter Verwendung dieser Verbindung erscheint daher wenig aussichtsreich.
