Abstract
Si le phéochromocytome peut être cliniquement asymptomatique, il peut dans de rares cas se preśenter comme une véritable urgence abdominale. Ces urgences sont le plus souvent secondaires à la nécrose hémorragique de la tumeur ou à un saignement abondant dans l’espace rétro-péritonéal. Les auteurs rapportent le cas d’un phéochromocytome révélé par une rupture spontanée de rate. Le mécanisme physiopathologique de cette rupture est discuté ainsi que l’abstention chirurgicale adoptée vis à vis du phéochromocytome au cours de cette chirurgie d’urgence. Dans un deuxième temps les auteurs discutent de la discordance observée entre taux de catécholamines et stabilité hémodynamique au cours de la chirurgie d’exérèse du phéochromocytome.
Abstract
Clinical manifestation of a phaeochromocytoma may range from no symptoms to an acute abdominal emergency. These abdominal emergencies are related to haemorrhagic necrosis of the tumour, or massive bleeding in the retroperitoneal space. The authors report a case of splenic rupture revealing a phaeochromocytoma. The mechanism of splenic rupture is discussed as is the conservative treatment of phaeochromocytoma during splenic surgery. The authors observed no correlation between plasma catecholamine concentration and blood pressure.
