Zusammenfassung
Nachdem in einer früheren Arbeit [2] schon über Chromosomen-Untersuchungen bei je einer Gamma-G- und Gamma-M-Paraproteinämie berichtet worden ist, werden jetzt die Befunde bei weiteren sieben Patienten mitgeteilt (zweimal Gamma-A-, dreimal Gamma-G-, zweimal Gamma-M-Paraproteinämie). Bei diesen sieben Kranken sind 750 Mitosen analysiert worden.
Knochenmarkkulturen von drei Patienten haben einen normalen Chromosomenbefund ergeben (s. Tabelle 2). Bei zwei Patienten mit Gamma-G-Paraprotein und einer Patientin mit Gamma-M-Paraprotein sind auch in den Blutkulturen keine abnormen Chromosomenmuster festgestellt worden. Die übrigen vier Kranken zeigen in 5–25% der untersuchten Mitosen Chromosomenanomalien in Form von großen dizentrischen Chromosomen, achrozentrischen Riesenchromosomen, Ringchromosomen und azentrischen Fragmenten (Tabelle 3). Teils findet sich ein überzähliges pathologisches Chromosom, teils besteht eine Pseudodiploidie oder eine Chromosomenzahl unter 46. Diese Veränderungen sind konstant, wie Wiederholungsuntersuchungen mit größerem zeitlichem Abstand zeigen.
Das in der Literatur mehrfach beschriebene Extrachromosom der Gruppe A haben wir in keinem Falle festgestellt und auch zwischen den verschiedenen Paraproteintypen keine Unterschiede in der Art der Chromosomen-Anomalien gefunden.
Mit den eigenen neun Beobachtungen sind z.Z. die Chromosomen-Befunde bei 49 Patienten mit einer Paraproteinämie bekannt. Die hierbei beobachteten Abweichungen vom normalen Chromosomen-Komplement gehen über bloße Zufallsbefunde hinaus, sind aber weder numerisch noch strukturell von denen unterschieden, die bei Tumoren verschiedener Art, nach starker Röntgen-Bestrahlung und bei Syndromen mit endogener Chromosomenbrüchigkeit vorkommen.
Summary
Seven cases of paraproteinemia have been studied cytogenetically (2 Gamma-A-, 3 Gamma-G-, and 2 Gamma-M-paraproteinemias). This study supplements a previous report of two cases (1 Gamma-G, and 1 Gamma-M-paraproteinemia), bringing the total to nine observations.
In 3 of the 7 patients of this report, bone marrow preparations were processed which showed a normal chromosomal complement.
Lymphocyte cultures were done in all cases: 2 males with Gamma-G-paraprotein and 1 female with Gamma-M-paraprotein had no chromosome aberrations. The other 4 patients presented 5–25% mitoses with numerical and structural chromosomal aberrations (Table 3). The chromosome number ranges from hyperdiploidy with a pathological extra chromosome to eu- and pseudodiploidy and to hyperdiploidy as well. The structural aberrations found are dicentrics, large acrocentrics, ring chromosomes and acentric fragments. All chromosome anomalies were consistent for a certain case, since they reappeared in repeated investigations after longer intervals.
The extra-A-chromosome reported in the literature in cases of Waldenström's macroglobulinemia could not be found in our cases. Neither was there a basic difference in the kind of chromosome abnormalities between the cases with different types of paraproteins
Including our nine cases, forty-nine cases of paraproteinemia are known in which cytogenetic studies have been performed. Summarizing the result of these studies, we believe that the chromosome abnormalities shown in this disease cannot be attributed purely to chance. Nevertheless, they are not highly specific. Rather they bear some similarity to those clonal aberrations observed in transformed tissue cultures, in tumors or even after excessive X-ray exposure.
