Zusammenfassung
Die Myelotomie nach R.Burkhardt, vorgenommen mit seinem für kindliche Verhältnisse verkleinerten Instrumentarium und mit nachfolgender Methylacrylateinbettung, erwies sich als wesentlicher Fortschritt der morphologischen Knochenmarksdiagnostik. Als Entnahmeort wurde bei Säuglingen die Mitte des oberen Tibiadrittels und die senkrechte Bohrung unterhalb der Spina ilica posterior gewählt. Mit dieser Methodik gelang es, die unklare chronische Anämie eines 10 Monate alten griechischen Säuglings als Myelofibrose aufzuklären. Bei einem zweiten Säugling von 8 Monaten mit Leber- und Milzvergrößerung, einer chronischen Anämie von rund 8 g% Hämoglobin und fehlenden biochemischen Befunden konnte ebenfalls eine schwere Knochenmarksfibrose nachgewiesen werden. Wahrscheinlich lassen sich die bisher sehrselten diagnostizierten Krankheitsbilder mit der verbessertenBurkhardt-schen Methode häufiger auffinden.
Summary
Myelotomy performed according to the method of R. Burkhardt and using his smaller size instruments adapted for pediatric practice, followed by methylacrylate embedding, was found to be a definite advance in morphological bone marrow diagnosis. In infants the material was obtained from the middle of the superior one-third of the tibia and by perpendicular drilling inferior to the posterior iliac spine. By using this method it was possible to diagnose the obscure chronic anemia of a 10 month old Greek infant as myelofibrosis. In another 8 month old infant with hepatosplenomegaly, a chronic anemia of approximately 8 gm% hemoglobin and negative biochemical findings, it was also possible to make a diagnosis of severe bone marrow fibrosis. By using the improved method of Burkhardt it will probably be possible to arrive more frequently at an exact diagnosis of these hitherto only very rarily diagnosed pathological states.
