Tubularized Incised Urethral Plate Urethroplasty in Severe Hypospadias

Abstract

Choices of procedures in proximal hypospadias are TIPU, onlay flap urethroplasty, inner prepucial flap/graft urethroplasty and two-stage repairs. Complications are less in TIPU and onlay flap urethroplasty than tubularized skin flap or skin urethroplasty. The neourethra constructed by urethral plate tubularization and spongioplasty is a near-normal urethra. The prerequisite for TIPU in proximal hypospadias is a well developed urethral plate of adequate width, minimal or no chordee and adequate size glans. These criteria are less frequently met in proximal hypospadias. But in selected cases, tubularized urethral plate urethroplasty is feasible. The main limitation to proceed for TIPU in proximal hypospadias is severe chordee. TIPU is feasible even in hypospadias with chordee provided the chordee is corrected, preserving the urethral plate. The urethral plate and urethral mobilization with or without dorsal plication may correct the curvature by preserving the urethral plate. A few technical modifications in chordee correction enhance the scope of TIPU in proximal hypospadias.
