Abstract
Reeds enkele decennia bestaat voor de dentale rehabilitatie van gedeeltelijk of volledig edentate kaken de mogelijkheid enossale implantaten toe te passen. Om deze implantaten te kunnen plaatsen moet er gezond en voldoende botvolume voorhanden zijn. De term ‘enossaal’ verwijst vooral naar de manier waarop dit tandwortelimplantaat in het lichaam is verankerd. Het gaat hier om een directe verbinding tussen het kaakbot en het implantaat. Dit principe heeft grote consequenties voor de bestaande of gewenste anatomie van het implantatiegebied. Mocht de aanwezige vorm van de kaak afwijkend zijn of de hoeveelheid bot – in driedimensionaal opzicht – onvoldoende en er zijn geen andere contra-indicaties voor implantatiebehandeling, dan zal eerst een aanpassing of opbouw (augmentatie) van het alveolaire bot moeten plaatsvinden. Deze botaugmentatie is overigens niet de enige vorm van pre-implantaire chirurgie. Ook de kwaliteit en kwantiteit van de weke delen rondom het implantaat zijn van belang voor een goede functie, optimale cosmetiek en lange levensduur van het implantaat. Indien er geen of onvoldoende gekeratini-seerde mucosa aanwezig is of de aanwezige mucosa te dun is voor een goed cosmetisch resultaat, kan het noodzakelijk zijn dat er gekeratiniseerde mucosa of bindweefsel of een combinatie van deze twee weefsels wordt getransplanteerd.
