Zusammenfassung
Allergische Reaktionen gegen Erdnuss (Arachis hypogaea, Ara h) beruhen auf IgE-vermittelten Sensibilisierungen gegen verschiedene Proteine. Ihre Stabilität und ihr relativer Anteil in der Erdnuss bestimmen das Risiko für bedrohliche Reaktionen: S2-Albumine (Ara h 2, 6 u. 7) sind eher relevant als andere Samenspeicherproteine (Ara h 1 u. 3) und (in absteigender Reihenfolge) Oleosine (Ara h 10 u. 11), das Lipid-Transfer-Protein (Ara h 9), das Bet v t-homologe PR-10-Protein (Ara h 8) oder Profilin (Ara h 5). Sensibilisierungen gegen die Speicherproteine Ara 1 und 2 sind charakteristisch für eine früh (im Kindesalter) einsetzende Erdnussallergie, die langfristig über Jahrzehnte persistieren kann. Ein gezielter IgE-Test, z. B. gegen Ara h 2 bei Verdacht auf eine Allergie gegen Erdnuss, kann die Risikoeinschätzung potenziellen systemischen allergischen Reaktionen erleichtern. Die Ergebnisse sind allerdings nur bei korrespondierenden Symptomen klinisch relevant. IgE-Sensibilisierungen gegen Erdnussextrakt ohne bedrohliche Reaktionen beruhen hierzulande häufig auf Bet v 1-bedingten Kreuzreaktionen (bei Birkenpollenallergikern), kreuzreaktiven pflanzlichen Kohlehydratepitopen (CCD) oder Profilinsensibilisierungen. Im Zweifelsfall lässt sich die klinische Relevanz nur durch eine orale Provokation sichern, zumal noch nicht alle Erdnussallergene (z. B. Oleosine) zur Diagnostik verfügbar sind.
