Abstract

Hypercalcemia is often associated with malignant diseases. The frequency of ectopic production of PTH as a cause of humoral hypercalcemia in association with malignant diseases is controversial. Some studies have suggested that patients with tumors and hypercalcemia have elevated iPTH in the serum or in tumor extracts (Sherwood et al. 1967; Benson et al. 1974), whereas others have been unable to detect iPTH in either tumor extracts or serum (Powell et al. 1973). Further studies have shown that humoral hypercalcemia is due to an altered form of PTH, which is unrecognized or only partially recognized by the radioimmunoassay (RIA) for PTH but which is still able to bind to the PTH receptor and is measurable in an in vitro bioassay (Goltzman et al. 1981). Up to now it has not been possible to distinguish between de novo synthesis of PTH by the ectopic tumor cells and the accumulation of exogenous PTH in the tumor tissue.
