Zusammenfassung
„Left ventricular noncompaction“ (LVNC) wurde seit Längerem als eigenständige Form der genetisch bedingten Kardiomyopathien gesehen, die durch eine Störung des embryonalen Kompaktierungsprozesses des Herzens bedingt ist. Jüngste Beobachtungen haben diese Definition infrage gestellt. Unter anderem wurden auch erworbene oder transiente Formen der LVNC beobachtet. Zwei neue MRT-basierte Studien haben nun gezeigt, dass der Nachweis eines Non-Compaction-Phänotyps nicht zwingend mit dem Auftreten einer Kardiomyopathie übereinstimmen muss: Eine Studie von Weir-McCall et al. zeigte, dass auch bei einem signifikanten Teil herzgesunder Patienten alle MRT-Kriterien für LVNC erfüllt sind. Eine weitere Studie von Daniele Andreini et al. wies nach, dass das Ausmaß der Trabekularisierung des LV nicht mit der Prognose von Kardiomyopathiepatienten korreliert. Diese Ergebnisse verdeutlichen, dass es einer neuen Definition des Krankheitsbildes LVNC bedarf, die nicht allein am Nachweis der Trabekularisierung des LV orientiert sein kann.
Abstract
Left ventricular noncompaction (LVNC) has been classified as an independent cardiomypathy caused by a defective myocardial compaction process during embryogenesis. Recent observations regarding acquired and transient forms of LVNC have challenged this definition. Two current magnetic resonance imaging (MRI)-based studies have now proven that the non-compaction phenotype has to be distinguished from a diagnosis of cardiomyopathy. A study by Weir-McCall and coworkers demonstrated that MRI-based criteria of LVNC can be proven in a significant subgroup of healthy volunteers. Moreover, a study by Andreini and coworkers showed that the degree of trabecularization of the LV does not correlate with the prognosis of LVNC patients. These results challenge the current definition and classification of LVNC.
