Abstract
The effect of pH-adjustment of three per cent 2-chloroprocaine (2-CP, Nesacaine MPF®) on the onset, duration, and spread of epidural analgesia and anaesthesia was studied in patients undergoing lower extremity surgery. Forty ASA physical status I and II patients were randomized to two groups. In a doubleblinded fashion, patients in both groups received an epidural injection of 15 ml of local anaesthetic (LA) solution via a Tuohy needle at the L3−4 interspace. Local anaesthesia for Group I was prepared by adding 3 mEq NaHCO3 to 27 ml three per cent 2-CP and for Group II was prepared by adding 3 ml 0.9 per cent NaCl to 27 ml three per cent 2-CP. Both solutions contained epinephrine (1:200,000). The pH of commercially prepared Nesacaine MPF® was 3.19 ± 0.02. The pH of the solutions used for Group I and Group II patients were 7.32 ± 0.01 and 3.27 ± 0.02, respectively. Times to analgesia and anaesthesia at the L2 dermatome were significantly decreased in Group I patients by 2.5 and 6.6 minutes, respectively. Likewise, pH-adjustment accelerated the attainment of maximum level of block by 2.8 min. No statistical differences were found between groups in the maximum level of epidural block, or in time to 2-segment regression. No precipitation of LA was observed in pH-adjusted solutions of 2-CP after 24 hours. We recommend the use of pH-adjusted three per cent 2-CP (Nesacaine MPF®) to accelerate the onset of epidural block.
Résumé
L’effet de l’ajustement du pH du 2-chloroprocaïne à trois pour cent (2-CP, Nesacaine MPF®) sur le dibut d’installation, la duree, et l’itendue de l’analgésie et l’anesthésie épidurale a été étudié chez les patients subissant une chirurgie des membres inférieurs. Quarante patients ASA I et II ont été randomisis en deux groupes dans une étude à double insu. Les patients ont reçu une injection épidurale de 15 ml d’un anesthésique local à trovers une aiguille Tuohy au niveau L3–4. Un anesthésique local pour le groupe I fut prepare par l’addition de 3 mEq de NaHCO3 à 27 ml de trois pour cent de 2-CP. L’anesthésique local pour le groupe II fut préparé par l’addition de 3 ml de 0.9 pour cent de NaCl à 27 ml de trois pour cent de 2-CP. Les deux solutions contenaient de l’épinéphrine (1:200,000). Le pH de la solution commerciale priparee de Nisacaine MPF® était de 3.19 ± 0.02. LepH des solutions utilisées pour le groupe I et le groupe II était de 7.32 ± 0.01 et 3.27 ± 0.02, respectivement. Les temps de l’analgésie et de l’anesthésie au dermatome L2 étaient significativement diminués dans le groupe I de 2.5 ± 6.6 minutes, respectivement. De même, l’ajustement du pH a accéléré l’atteinte d’un niveau maximal du bloc à 2.8 minutes. Aucune différence statistique ne fut trouvée entre les groupes dans le niveau maximal du bloc épidural ou dans le temps de régression. Aucune précipitation du LA ne fut observée dans les solutions où le pH fut ajusté aprés 24 heures. On recommande l’utilisation d’un ajustement du pH afin d’accélérer le débit d’installation du bloc épidural.
