Resumen
Se revisa la experiencia publicada sobre la utilización de la radioterapia en el cáncer gástrico. Se describen los resultados terapéuticos en la enfermedad irresecable, también cuando la radioterapia se combina con la cirugía, sea después o antes de ésta, y las maneras especializadas de administrarse (radioterapia intraoperatoria). Concluimos que, según los resultados del estudio del Intergroup INT 0116, la radioquimioterapia después de una cirugía radical tendría que considerarse en todos los pacientes de alto riesgo de recurrencia. Por otra parte, hay temas no resueltos en esta enfermedad que hay que seguir investigando, como son el tratamiento en los tumores irresecables o las indicaciones de la radioterapia intraoperatoria. La radioterapia preoperatoria, preferiblemente combinada con quimioterapia, es un tópico interesante, sobre todo en los tumores irresecables o en el límite de la resecabilidad.
Abstract
The experience published on the use of radiotherapy in gastric cancer is revised. The therapeutic results of the unresectable disease are revised, also when radiotherapy is combined with surgery, whether before or afterwards, as well as the specialized ways of administering it (intraoperative radiotherapy). According to the results of the study of Intergroup INT-0116, we conclude that radio-chemotherapy after radical surgery should be considered in all patients with a high risk of recurrence. On the other hand, there are unresolved subjects in this disease that still have to be investigated, such as the treatment of unresectable tumours or the indications of intra-operative radiotherapy. Preoperative radiotherapy, preferably combined with chemotherapy, is an interesting topic particularly in nonresectable tumours or those on the limit of resectability.
