This may particularly be true for patients with CVID without associated clinical and laboratory characteristics (Table II) or other PADs in which ILD has not been reported, such as XLA, and risks of repeated CT scans outweigh unlikely benefits.145-147 Conversely, in CVID patients with the associated clinical and laboratory characteristics, or in those with monogenic immune dysregulation disorders associated with ILD, presence of respiratory symptoms should trigger evaluation of ILD.

There has been a reluctance to overexpose patients to x-rays based on reports of radiosensitivity in CVID65-67; however, improved safety and similar sensitivity may be possible by using magnetic resonance imaging when more frequent imaging is needed.68 In addition, there are important ongoing initiatives in the standardization of HRCT assessment for antibody deficiency (http://www.chest-ct-group.eu) and prospective analysis of HRCT of the chest for patients with CVID across time in ongoing studies.
